Our mental health program provides quality in-home psychiatric and medical care for primarily home-bound patients who have been recently hospitalized, have a new or changed Axis I diagnosis, and face one or more of the following challenges:
- Impaired thoughts such as hallucinations and/or delusions
- Signs and symptoms of depression
- Medication noncompliance
- Confusion, dementia, alzheimers
- Change in psychotropic medications
- Anxiety, Fear, Agitation
Our interdisciplinary team is comprised of a Psychiatrist, Psychiatric Certified RN, Licensed Medical Social Worker, Occupational, Speech and Physical Therapists and home health aide. All team members work to maintain a viable relationship with the referring Psychiatrist or the Medical or Mental Health Facility by reporting any behavioral or physiological changes experienced by the patient. Such a relationship assures continuity of care.
Our overriding goal is to provide the best possible care to those whose mental health can best be treated in their own home.
We accept Medicaid, Med/Waiver, Worker’s Comp, Private Insurance, and Private Pay.
